Sunrise Transitional Services provisions are completely gender specific, offering accommodation separately to the following young people:

Individual Male and Female homes

These homes are structured to ensure positive outcomes, including the reduction of risk taking behaviour and to ensure that the young people in our care are focused on achieving the necessary skills to live independently.

An emergency bed is available within our individual homes which can be used by any authority for a period of a two week placement.

Each transitional home is staffed 24 hours a day by a highly motivated and professional team and managed by a qualified social worker. Our homes accommodate no more that 5 young people. These small groups allow the young people to receive targeted support and suitable supervision, eliminating the issues of lack of engagement whilst ensuring the young people do not feel overwhelmed and or crowded.

The staff team within the homes supports each young person with all appointments including health, education, professional and personal such as registration with local doctors, dentists, opticians, education meetings and interviews, LAC reviews, medicals, placement planning and moving on into independence.

We work with a ‘Team Around the Young Person‘ model of support to provide the best outcomes and future for all young people in our care. All staff will contribute to creating the plan. We will consult with the young person’s placing social worker over the Pathway Plan and work with them as well as other appropriate outside agencies to ensure the young person is meeting their targets.

The support team will actively help and support young people into living healthy life styles by advising them in healthy eating, encouragement into sport and recreational activities and helping them to appreciate the importance of healthy living.

We will actively encourage and support young people into further education or employment by making contacts and helping prepare young people for interviews and CV’s.

The support team will provide direct support to the young people in gaining their independent living skills that will include support in cooking and budgeting, home and personal hygiene, an awareness of neighbours and community, use of domestic appliances such as Iron, washing machine, cookers and other appliances.
