Sunrise Sixteen Plus Outreach Services

Sunrise Sixteen Plus outreach service is a bespoke service designed to meet the individual needs of young people who can benefit from our services.

This service is offered to young people currently placed in our transitional services that require additional outreach support. Once a Young Person is ready for the next step, Sunrise sixteen plus offers a follow on service in order to fully support a young person’s transition into the community when they are ready to live independently.  We can support young people in all aspects of moving into their own accommodation using different agencies.

When the young person moves to their own accommodation the level of support can be reassessed and support offered by our outreach service whilst they settle into their new home.

Through the development of bespoke Care Plans, pathway plans, specialist interventions and appropriate support methods young people will be fully prepared and equipped with the skills and confidence to manage their lives when they leave the transitional home, and indeed when they leave care.

All young people will be supported through a series of ‘Life Skills’ sessions with key workers preparing them for independent living. The key-workers play a significant part in this process, preparing the young person for their change of placement and supporting them during the initial settling in period. This process may include accompanying young people on visits to new placements and offering appropriate support and guidance to ensure this is a smooth transition which limits any distress or upset.

At Sunrise Transitional Services every young person will have the opportunity to be fully involved in creating the plans made for them. This will be a collaborative process between the young person, staff and their social worker. An initial assessment will be undertaken to ascertain the current skills and abilities of the young person. Each young person will have a pathway plan. The formulation of this will be a collaborative process between the young person, staff and their social worker. Relevant information may be gathered from other agencies/professionals involved in the young person’s care.

These plans will include:

  • Health matters including Sexual, Physical and Emotional
  • Education and Employment
  • Independent living and life skills
  • Budget management incorporating rents, bills and welfare benefits
  • Social skills, self-esteem, confidence and motivation
  • Legal/immigration matters
  • Long Term Accommodation
